Good case, and reminds me of pyoderma faciale. Would start on minocycline/doxycycline po and a topical clindamycin-benzoyl peroxide combination. Would also do hormone profile to rule out PCOS, androgen-secreting tumor etc.; based on results, can consider cyproterone acetate or spironolactone.
ReplyDeleteThis is late onset ordinary acne vulgaris which is a common problem seen in this age.So if we do investigations for every patient with this problem to exculd pcos or other androgen secreting tumors,we will need labs open day and night to do investigations.History and examinations with good clinical experience are enough to establish the right diagnosis.Treat as case of acne vulgaris and follow up will tell us the end of the story
